What is Hallucination?

When an AI model generates plausible-sounding but factually incorrect information. All LLMs hallucinate to some degree. According to the LLMCheck index, larger models with higher capability scores generally hallucinate less frequently, but no model is hallucination-free. RAG (Retrieval-Augmented Generation) reduces hallucination by grounding responses in retrieved source documents.
